As a courtesy we assist our patients in the filing of their dental insurance so please bring your insurance information for us to have on file. You may want to call your insurance company to find out what your coverage is for treatments in our office prior to your visit. Insurance reimbursements will be sent directly from the insurance company to the policyholder.
The guarantor for the account is responsible for the all payments to the office, as we do not directly participate in any insurance plans. We will make our best efforts to work with you if a payment plan is necessary for your continued oral health. Please let us know if you have questions about our financial policies prior to your treatment.
Payment is due at the time of service. We accept cash, checks, debit cards, and credit cards such as Mastercard, Visa, Discover, and American Express.
Dr. Roz recognizes that everyone’s time is valuable and prides herself on running appointments on time. Your appointment time with Dr. Roz has been scheduled exclusively for you, so we ask that you please respect Dr. Roz’s and other patients’ by arriving on time for your appointment.
To avoid running late and prevent long waits for patients, any patient who arrives more than 15 minutes late for their appointment time may not be able to be seen and may need to reschedule their appointment.
Please let us know as soon as possible if you are going to miss an appointment. Any cancellations less than 24 hours before an appointment will result in a cancellation fee.
